FEES AND PAYMENT POLICIES

Below is a listing of my usual fees, as well as fee and payment policies, to assist you in planning and understanding of financial obligations.

*Co-payments/Co-insurance:* If you have insurance with which I participate or accept assignments, your portion of the fee will be expected at time of service. Please consult with your insurance carrier ahead of time to determine your copayment. Sliding scale available for consideration for those with no or insufficient insurance. REDUCED FEE PROGRAMS Couple’s Therapy Reduced fee hours can be scheduled from 9:00 a.m. to 3:00 p.m. Monday through Thursday each week. Dr. Fyfe offers a program for couples who are without insurance or who have high deductibles making therapy difficult to obtain. Couples sign a statement acknowledging that they do not have health insurance or submit evidence of a large insurance deductible to qualify for a $65.00 per hour fee for couple’s therapy. This rate represents over a 60% reduction in Dr. Fyfe’s normal hourly rate Parenting Plan Consultation Dr. Fyfe offers parents an opportunity to de-escalate conflict and create a parenting agreement. Parents sign a statement attesting to their unemployment or significant financial difficulties which make full fee for services impossible. Parenting plan consultation is provided at 50% fee reduction for a minimum of two hours per visit ($175.00). Parents can utilize up to four hours of reduced fee service.
