Welcome to Columbine Counseling Center,
P.C. I want our communication to be open and for you to feel free
to ask questions. The following information tells you about my
services, and policies. Thank you for selecting me to serve you.

EMERGENCIES
Coverage for emergencies is available at
all times. If you have an emergency during off-hours, call our
office and follow the instruction given in our voice mail system.
Fees for emergency care may not be covered by your insurance.
You may also call West Pines at Exempla Medical Center (303-467-4080),
or Presbyterian/St. Luke’s (303-869-1999), for emergency
assistance.
RECORDS
Your file is a confidential record of treatment.
When you conclude work with a CCC therapist, your file remains
on site for two years. It is then transferred to a Denver storage
facility. The exception to this procedure would be if your therapist
leaves CCC before the two-year interval. In that case, your file
may be transferred to storage sooner. Once in storage, a $20 fee
will be charged for file retrieval, except when returning to see
Dr. Fyfe for further treatment.
TELEPHONE CALLS AND MESSAGES
Support staff is generally available from
9:00 a.m. until 12:00 p.m., Monday through Friday. At other times,
the phones are transferred to voice mail messaging. If your call
is an emergency, please follow the instructions given to you on
the voice mail message. Dr. Fyfe will make every effort to return
calls each day in a timely manner.
APPOINTMENTS
During your first appointment, we will make
arrangements for further sessions. Appointments are times reserved
for you. Please Note: Failure to keep an appointment without notifying
us will result in a charge of one-half fee for cancellations with
less than 24 hours notice, and full fee for missed appointments
without notice. (The voice mail messaging is available to take
cancellations after office hours or on the weekends.) This allows
us to make the time available to others who need it.
RESPONSIBILITY OF PAYMENT
(Dependents)
It is the responsibility of the parent/guardian to pay for a child’s
treatment. It is the responsibility of that parent to provide
CCC with all insurance information necessary to collect payment
from insurance. If this information is not provided, the parent
bringing the child will be responsible for complete payment. If
the custodial parent expects a third party to pay some or the
entire fee, he or she must deal directly with that person. Until
the signed registration form designates acceptance of payment
to a third party, the parent bringing the child to the office
will be responsible for payment.
